Kenape Saye Musti Kurus
So much for that…Happy Ramadhan to all…a month that all Muslims are waiting for various reasons…org berumur nak banyakkan amal ibadat…yang mude2 nak kuruskan badan kononnye…termasuklaa aku nih yg mmg dah berjaya mencapai tahap obesiti…ughh!! Walaupon lambat wish..takpelaa…selamat tinggal puasa sekali…hohoho!!!
This month is the most suitable and rightful time to trim off most of the unwanted fat…ey…fat pon ade gune jugak tau…kena laa simpan sket..bak menteri besar melaka cakap kene ade kuota…babi kat linggi pon ade kuota..begitu juga dgn lemak di badan kite nih!!
1. Convocation Day
Wahh…lagik 6 bulan nak final exam..pastu kalo pass dah bole berangan nak konvo…member2 lain sume dah merase…and for sure I want to look hensem and slim during that day…kalo tak jubah konvo tak muat maa…perut besau!!so…this is the main reason why I should be slim…it’s for convo dude!!
2. Plantar Fasciitis
Yada yada…seperti yg diketahui…since early this year I’m started to have pain on the heels…most severe on the right one…spot diagnosis la brader…plantar fasciitis = inflammation of the plantar fascia…suit btol laa…muda remaja dah dapat…obesity punye pasal laa…until the stage I’ve got pain also at the tendon behind the heels…lagik satu spot diagnosis…Achilles peritendinitis…kalo pegi ward round pagi2…mmg hell gile ahh..rase nak tercabut kaki…tapi sekarang dah ok sket laa…pakai comfort shoes ngan kurangkan makanan..ewah!!
3. Kacak Bergaya
I noticed huh…since I got fatter…no girls wanted to flirt with me laa…hahahah!!mcmlaa mase kurus dulu ade…awek pond ah bising2 apehal makin gemuk…senang hati sgt ke belajar medicine sampai makan banyak…huhu!!kalo ade pon awek keling mane ntah dok usha…kamon laa…bollywood aku layan kareena kapoor dan seangkatan dengannye…hahahha!!

Since masuk medicine…adelaa gained dlm 20kg…5kg per year…not bad jugak laa…target sekarang nak kurangkan 10kg jek…nampak sket…tapi susah woo…kalo diet senang jek nak turun 3 4kg..tapi bile makan lebih sket jek…senang btol naik…rase cam tak berbaloi pulak diet sebelum nih…aiii!!

Cakap pasal rambut nih...tak tau nak cakapla sebenarnye...2 3 menjak nih baru perasan macam ade yg tak kena...rase mcm normal jek...tapi bile pagi2 sikat rambut tuh...mmg jelas dan nyata perubahannya...ingat sementare tapi makin lame makin menggila..heh!!but I'm still consider it to be a normal phenomenon cause the loss area is typical of temporal recession of a man...hahah!!
Yang tak tahannye, dah kurang kat atas...yg bwah pulak banyak...opps!!bukan yg bawah tuh... atas sikit dari yg kat bawah tuh...heheh!!ape2 jekla...aku tgh bengung nih nak exam..besela!!nak tgk Mlaysia kalah bola lagik nih..tak tau sempat bace ke tak...wah!!

I was in a school band since form 1...I choosed to play a C flute because I like seruling buluh and those who playing it look smart and handsome...hahah!!what a stupid reasons!! It took 2 years for me to master the fingering and got the pitch right and that time I can play all the notes in 4 octaves just in one minute!! I never regrets playing it since the sound was so nice and peaceful... all my stress and problems went away thru its sounds and melody [and of course all the stress and probelms came back again after I stop playing it..haha!!]...Now, it has been 5 years since I left it and maybe I have lost the skills and the techniques...aii!! Sure tokwan dah lagik power dari aku...die ade flute sendiri bole laa practice...ceh!!

I also joined my school rugby team...eventhough I was not a good player and represents the state...I did enjoy my training and learning about the game...we were so proud being the ruggers or better known as STAR CobraTasha...we were very enjoyed and loved this game!! "the rough game played by gentlemen.." - that what was arwah Nawi often said to me...

I have lost my campus life...no more hot UiTM chicks to flirt around and no more active social life...now it's all about patient and medicine...boring!! Thinking about it everyday make my brain sizzled...I'm at the point of feeling nauseated when going to the ward...fuh!! It's very hard to hang around with my friends from the old school aw well as my non-meds friends...poor me!! My girl also has to suffer..being so lonely and missing me a lot...hu!!
I really lost 'them'...and I'm always hoping to get them back...damn I really want it!!

New Perspectives: Urban Health
Urbanisation refers to a change in size, density and heterogeneity of cities. According to WHO definition of health, it is a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity. Therefore urban health refers to the health of people living in the cities (very simple straight forward definition….heh!!).Malaysia has experienced a rapid industrialization and urbanization over the past 50 years. Below are some facts about urbanization across the world and in Malaysia.
The urban population has increased from 37% in 1981 to 50% in 2000 (Roziah Omar, 2005).
United Nations projects that 61 percent of humanity will live in cities by 2025.
Majority of the world’s population will live in urban areas by 2007 (Lancet).
About 75% of Malaysian population will be living in cities and towns by 2020 (NST, August 6 2006)
With rapid development of the city, especially in Malaysia, we can’t run from the problems that can arise from the urbanization processes. Most of us know it very well (common sense huh!!). The problems include physical and social as well as mental health. They are as follows:-
Physical
Air pollution, noise, and nuisance
Motor vehicle accident (anda mampu mengubahnya)
Pedestrian injuries and fatalities
Housing – squatters, high-rise building, overcrowding, homeless
Climate changes – El Nino, La Nina, green house effects
Physical inactivity – Diabetes Mellitus, Hypertension, Obesity
Water quality & quantity (dahla rakyat membazir air, nak jual kat Singapore lagik…SYABAS!!)
Waste disposal and sewerage (Oh tandas…berkualiti!!)
Social
Migrant workers – Indon, Bangla, Myanmar, Nepal (datang buat menyemak, buat masalah jenayah, sebar penyakit pastu kahwin ngan org Malaysia)
Occupational diseases
Stress
Crime and abuse
Sexual transmitted diseases (papa jahat!!)
Poverty
Drug addiction (hidu gam ngan taik lembu pon balun jugak…layan!!)
Psychosocial
Divorce (mama tahu papa ligat!!)
Long commutes (that’s the way we jam)
Road rage (yeyeh!!)
Decreased leisure time (tapi papa sempat buat jahat lagi)
Fear – crime, claustrophobia, post-traumatic stress
Loss of sense of community – TV, computer, PS (baru nak kempen ‘kenali jiran anda’)

So people…what should we do?? Continue to live this way?? Is this what should we pay for freedom, after 50 years of independence? Banyak lagik tak cerite atas tuh..malas nak elaborate sume…leceh!!

Pushing the limits...
What the previous, present & future doctors can’t do?
Since many centuries ago, the medical fields have expanded tremendously, from only a few fields of interests including medicine and surgery until the super subspecialty that we can see today. You can name any specialty and you can find a lot of new inventions which they said for the best interests of the patients. The invention of new surgical techniques and development of new, effective drugs have significantly increased the success rate in the treatment of the diseases leading to a better survival and improving patients’ quality of life. But there is one thing that they should not forget…
Our first rotation in medical specialty postings were not so fantastic, it has revealed a lot of our weakness and mistakes that we’ve done before. It doesn’t took a short time for us to develop such complications; the first group to be tested on a new integrated, improving curriculum that they’ve claimed to be among the best in the world, in parallel with the vision of leading the medical profession and influencing the practice worldwide.
The second round would be the respiratory medicine and neurology. My first day in IPR KL which is the tertiary center and the national referral center for the respiratory diseases was a good experience. During the orientation in the morning with the ward sister, one patient has collapsed and the resuscitation team immediately attended the patient and tried their very best to save him and he was intubated. His condition was not so good but they managed to control the acute situation. We continue our ward works, clerking patients and practice the physical examination. After we finished performing Zuhr pray in the afternoon, suddenly there was high-pitched, continuous alarming sound that we know what it meant. It came from the continuous cardiac monitoring machine that showed an asystole rhythm of the patient’s heart. We can saw it very clearly through the window since the surau is situated just behind the ward. The patient passed away...
We just proceed with our scheduled timetable and in the afternoon until one more patient collapsed and they’ve done the same thing. Unfortunately the same outcome they’ve got. The resuscitation failed and the patient was not survived.
I suddenly realized that how great the technology has improved, how good the medicine was, and how hard we try, it was not us that determined the patient’s fate. Some of us thought that the doctors are the healers, the God; they can recognize the disease and start the appropriate treatment and then follow them up regularly and eventually the disease is cured or controlled. But in the end, it is God (Allah) that decides the entire outcome. He is The Healer, and we are just the ‘equipment’ that serves as a medium between Him and the patient. That is the fact and we, especially the future doctors should never ever forget about that, not even once!!

That was kind a bad closing statement when you were in the exam…after you’ve been struggling to find for the answers…then you started to think whether your ward work or your cirrhotic brain can help you to reason out your answers…not mentioning about the over sympathetic drive that make you feel so disgusting…very dry mouth and lips, palpitations with impending arrhythmias, hyper hydrosis prominently seen at the frontal area with a few drops started to wet your face…huh!!
"Not me!!"
Then few days before the exam…they behave, walk and talk like a zombie…reading so many books and notes like there is no tomorrow….this last minute study is a very popular style and have been applied by all students across the nations!! When the day come, all of them will struggle to vomit and retching out all the facts they’ve read, all the things they’ve seen and all the knowledge that they’ve know. The results then been released and those who were successfully vomited out the sufficient things will have a good grades and few will fail at least one of the exam components.
The question is…does the result reflect their true ability, their true understanding or their actual skills in mastering the knowledge? What they’ve already vomited out make their ‘stomach’ became empty? Ward works, seminars, lectures, on calls, so on and so forth…can they retained all those facts for a long time? Why when they’ve been asked again after a period of time…they seem to know nothing about that, like they’re never seen or heard it before? Where does all the hard works go?
I am one of those people…after 2 months having all sort of so-called hardship…learnt a lot of the things that we must know and understand earlier...and realizing more weakness and defects that we have and yet there was very few that can be improved. But same things happened…again and again!! Our beloved dean said we must study for the exam and yes we do followed his advice with a little misunderstanding…we thought it was for the end of posting examination but what he meant was for the final professional exam 3!! The most difficult…and most unfair exam…on earth!!
What should I do? What should we do? What should they do? ...to make sure these things won’t happen again…it was really a heartbroken moment when we realize that all the hard works went into the drain after the exam…provided they’ve really work hard to get as high as they can…huh!! What a shame!!
I’m hopeless!! Help me!!

Living in a sky high expectations with a down low motivation
That was what most of the lecturers said to us...when they were looking at our previous...and current performance in the various postings that we have been thru...it is not really good if we want to compare with the juniors whose the curriculum has been refined and tuned well with a near complete facilities and teachers...it is not a matter of jealousy or any inferiority complexes but how much we can do to reach the standard within a short period of time...
Don't know how to put the feelings in words...it is hard to say...when we keep on struggling to live in a high expectation with a down low motivation

"Tonite...we will dine in hell!!"
During this posting...I learnt a lot of things...about myself and about my future duty...now I know what I should do when they approve me to be safe and competent to serve the community...in summary it is called "The Clinical Work Processes"
Though it was too late to start from the beginning...but at least there is lil' time left to improve...in whatever subject...from the normal human structure and function until it become abnormal and diseased...all of them need to be mastered in this short period of time...because if I cannot met the standard...I am no better than the MA (nih bukan nak kutuk MA, tak terniat langsung...serius!!)

Roughly around 300 days left before we come to the most difficult and most unfair exam on earth...the final professional exam 3!!! It going to be the last and the best 300 days to get (lot of) things done and make the dreams come true...to lead the medical profession and influence the practice worldwide...yosh!!
Help me!!! I'm stucked!!
All works no play makes Jun a dull boy
That was what my professor in neurosurgery said when we started our surgery posting...heh!! It may seem funny or clumsy to you...but it means a lot to me...since we have to be dependent when in the ward with less attention from the lecturers...year 3 students are not the reason why we should stop our clinical works in the wards...not enough patients...no cases...it's all stupid reasons to be told!!
But that is not the issue...during this "enjoyable" postings...we have a free time to do sports...went back home after 5 then quickly rushed to the hall to play badminton and pingpong...after a long time...finally we had a good time to sweat our obesed body...don't know how many kilo that we've put when we were not active...some of us already doubled their size compared to when we entered the medical school...and of course most of us have already loss their beautiful body shape...believe it!!
It was great to exercise and get tired...rather than went back home with a lot of stress and walk like a zombie...not interested with the surrounding...sleep in the room until the next morning (some would scream "alamak...aku tak study ape2 lagik!!" when they woke up)...and some may have planned to lose a few kg after struggling with the rackets and shuttlecocks...yosh!!
I hope I also can lose few kg...because I am getting bigger and wider...nak melancap pon dah tak bole jadik tinggi...sebab epiphysial plate dah closed...hahah!! It was easy and fast to gain weight...but it's hard to lose it even a kg...fuhh!!
MCQs strikes back!!
Finally...it takes a failure in an exam for us to start back the long lost forgotten study group...hahaha!! Very typical of Melayu MS...bile menari dongak ke langit...langkah sumbang salahkan bumi...silap sedikit tak dipeduli...yeh yeh!! Bile dah berjaya sikit...ape yang ade sume ditingalkan...sebab rase diri tuh hebat sangat...kesian!!
Tapi lebih baik fail sekarang...daripada mase PRO 3 nanti...at leat there is still room for improvement and the time left can be used to strengthen our basic and optimizing our knowledge...yosh!!
To fulfill the vision of being the leader of medical profession and influence the practice worldwide...it needs a lot of effort and sacrifice. We need to have a good knowledge, extraordinary medical skills and excellent teamwork to make the dream come true.

One step closer to the end...
That was what my professor in medicine said when we failed our medical posting...hola!! It is quit tough to set the standard for those below us since we are the first to experienced all these experiments...like a guinea pigs in the labs...hahaah!!
Clinical practice during the 4th year is much more different from the 3rd year...as an introduction to clinical medicine...3rd year students are expected to take a good, detail and comprehensive history and performing complete physical examination in order to arrive at the differential diagnoses...but in year 4...it is one step higher...we have to know what necessary, appropriate and useful investigations and formulate a detail short-term and long-term management of the patient...fuhh!!!
We failed and performed very badly during the medical posting because we're still in the year 3 standard...a lot of new information that we must know in a very little time...plus all the knowledge about the diseases that've been learned were already forgotten (kindly refer my previous post: 100% reason to remember the name)!! Now you know that you have to know all these when you proceed to year 4...and year 5!!
Once again, they've asked for a detail management and drug dosages for all the problems...in the exam!! Now we realized that is the current standard that we have to achieve...but it is already near the end of the year...which means next year would be a double burden to achieve the higher standard...ai!!
To all of you my friends...be prepared!! If you fail to plan, you plan to fail!!

100% reason to remember the name
I can still remember that sentence very clearly...told by my professor in anatomy class...heh!! It is alraedy 4 years since he had said that sentence...referring to the above statement...it is true that patient don't come with 50% of the illness...they come with 100% of it...applying it to our responsibilities...which means we have to know everything about the human structures, functions and processes...not to forget what disease(s) can arise when those structures, functions or processes became abnormal...fuhh!!
"Ahh...lulus pon dah cukup ok dah!!"...that is what we always said..but our attitude is wrong...and unacceptable!! It is true that we've passed all the exams...nothing great about it when it come to applying the knowledge to the real clinical scenarios...we can't relate it to the patient...this is because we don't have the remaining 50% which is very important to make a dignosis...
Another fellas will say "We don't have to know all...just the principles of those diseases"!!--> yes, you're right...very true...but the problems is...the 50% that we have is only the facts about the diseases...definition, aetiologies, risk factors, epidemiology...memorizing the clinical symptoms...investigations and its management...BUT we don't know the pathophysiology of it(how the disease developed and affect our body)...don't know how to relate it to the clinical features that the patients have...and then after quite sometime...when another patient come with the similar symptoms...we've already forget...
Fact is fact...it can be read later...BUT the basic foundation must be there...another professor has said "If you got your basic principles strong...nobody on earth can shake you..never!!". All those craps saying by the old mans are true...it is just that we can't appreciate it...hahahah!!Poor me!!
Now...no much time left...only another year left...so much works to be done!! A lot of books to be read..and revised!! A lot of procedures to be performed...and practiced!! A lot of patients to see...and to be examined!! Again and again!! Plus...melayu mudah lupa!!hahahah!! What a shame to a Malay MS...
Don't know what to say...what to do...what to think!! Aku terpaksa menerima padah dan natijah atas ape yg aku dah lakukan...belajar hanya nak pass...dan hanya untuk exam!! I'm ready to face all the consequences...and for the next postings...I'll never do this again... not even once!! Cewahh...
